What is HealthLink?

Funded under CMS CPT Code 99454 HealthLink Insight™ is a multi-vector monitoring, analytics and alerting service developed specifically for providers, health systems, payers, and stakeholders treating Chronic Condition Management (CCM) and Principal Care Management (PCM)  populations.

The Elevator Pitch:

HealthLink Insight™ is a novel model and service designed for providers, stakeholders and payers.  We've simplified the patient experience, lowering cost and complexity, by eliminating the apps, tablets, and WiFi.  Readings go from device (bp, glucose, weight, SPO2, etc.) to hub automatically; in turn uploaded to the HIPAA Cloud over encrypted AT&T or Verizon wireless networks.

Providers, including PCPs, Specialists, Home Health, TCM and CCM teams are each connected to the patient in a 1-to-many model with each stakeholder alerted according to the the stakeholder's selected acute, longitudinal, adherence and plan-of-care compliance criteria appropriate to their view of the patient.    

Alerts are timely, tailored, and actionable -enabling proactive identification and intervention.   Stakeholders choose their preferred notification outlets including secure message systems, E-Mail and/or custom SMS delivery.

This unique model supports custom patient panels and insight with 1:many (patient-to-providers) and many:1 (providers-to-patient) capability, inclusion of up to ten stakeholders per patient, and unlimited use all at no additional cost.

HealthLink was designed for simplicity for patients, providers and stakeholders.  

HealthLink has removed away the complexities. There are no apps, no WiFi, no tablets, and nothing to recharge. Patients need only take the biometric readings such as Blood Pressure, Weight, Glucose, SPO2 consistent with their provider’s plan of care in the treatment and management of Diabetes, Hypertension, Obesity, Heart Failure and COPD.

Readings are  automatically transferred via Bluetooth to a HealthLink partner- hub, a palm-sized device that is wall powered and relays received readings to the HIPAA Cloud via an encrypted upload over AT&T or Verizon’s nationwide wireless networks, against acute, longitudinal, adherence, and plan-of-care guided compliance criteria tailored by the provider or stakeholder for that patient.

For the first time, HealthLink enables one patient to be simultaneously monitored by multiple stakeholders, each with their own criteria, and at a single-per-patient cost covered by CMS under CPT Code 99454 and presented in provider-selected patient panels.
