Specialists: PCM was Designed for You

In Four Bullet Points:

• CMS approved PCM in 2020 - Billed to G2064, complements 99454 and 99091.
• PCM services are for patients with at least one chronic high-risk disease lasting 3+months, the focus of a care plan, severe enough to place the patient at risk of hospitalization or been the cause of a recent hospitalization, ans where the patient requires development or revision of a disease-specific care plan; adjustments to medications.
• HealthLink services are covered by CPT 99454; 
• Anticipated FCF can reach $1,200-$1600/pt/year inclusive of 3rd party, RN-led weekly monitoring and review of each of your enrolled patients. 

Obtaining Insight and Improved Outcomes was never so easy:

1. Identify qualifying patients & complete the 1-page enrollment form
2. Review patient status and results consistent with 99091, G2064 requirements
3. Engage patients as appropriate (telephone, telemedicine, in-person) consistent with care management needs and bill E/M codes
4. Invite referring providers, PCPs and affiliates for monitoring at no additional cost to you or your practice - promote goodwill and share insight for better patient health outcomes and QPI. 

And, if you serve populations that are part of FQHC or RHC programs, effective January 1, 2021, PCM service is expected to be available in support of those populations!
